The CCS program provides diagnostic and treatment services, medical case management, and physical and occupational therapy services to children under age 21 with CCS-eligible medical conditions. We submit requests for services on behalf of the families to determine eligibility. If they are eligible, we provide case management and assistance with referrals for care and specialty equipment and medications. Quarterly, we sit in on the CCS Medical Therapy conferences for children who are determined to need further assistance with equipment and such to advocate for them.
